ACO Governing Body Structure
The Collaborative Care Partners Governing Board oversees all of the operations of Collaborative Care Partners and reports on its ongoing operations and outcomes to The MetroHealth System Board of Trustees. The Collaborative Care Partners Governing Board includes members that represent ACO REACH Participant Providers, ACO REACH beneficiaries and community stakeholders. The Executive Vice President and Chief Clinical Transformation Officer of The MetroHealth System serves as the Chair of the Collaborative Care Partners Governing Board and the remaining members include primary care and specialty physicians, health system administration, a Medicare fee for service beneficiary who receives care in The MetroHealth System, and a community stakeholder.
