Track My Health: Record and Share Information with your Care Team
Track My Health allows you to record information in your medical chart that both you and your doctor can see.
You can share data you track on your device - such as heart rate, step count, or blood pressure - with your care team at MetroHealth.
Get started with Track My Health. It’s easy!
- Ask your provider if Track My Health is right for you. Your provider has to enable this by placing an order.
- Create a MyChart account.
- Connect your fitness tracker to your MyChart account.
Setting Up Your Fitness Tracker
Fitness tracker linked to the “Apple Health” or “Google Fit” mobile app
- Download the MyChart mobile app to your smartphone.
- Log into MyChart mobile app and navigate to the “Track My Health” menu.
- Tap “Connect to health”.
- Follow the prompts to connect your fitness tracker to your MyChart account.
Fitbit or Withings Fitness tracker
- Log into the MyChart website through a web browser and navigate to the “Track My Health” menu.
- Click “Connect My Tracker”.
- Click “Connect account” next to the fitness tracker you will be using.
- Follow the prompts to connect your fitness tracker to your MyChart account.
FAQs
What if I get a message that one of my vitals is outside the normal range?
Readings entered in Track My Health may only be reviewed during scheduled visits. If you need immediate attention, dial 911 or seek medical help from the nearest hospital or emergency treatment center.

- Search for "MyChart"
- Download the app
- Open the app on your mobile device and choose MetroHealth as your provider
