Jonathan Lever is a Program Manager at Better Health Partnership. He has experience implementing large cross-sector population health initiatives and oversees the design, implementation, and evaluation of Better Health’s portfolio of programs. He leads the operations of the Better Health Pathways HUB, an evidenced base community care coordination model that helps high risk populations address their health, social, educational and economic needs. Jonathan works closely with the community health workers who see clients in the HUB. He leads implementation of the Children’s Health Initiative programs which include developing an electronic bi-directional referral and feedback system between four health systems and United Way 2-1-1. He also manages a multi-health system project to decrease extreme prematurity in Cuyahoga County. He helped implement the evidence-based best practice for hypertension management in the Racial and Ethnic Approaches to Community Health (REACH) grant from the Centers for Disease Control serving in both project manager and practice coach roles.
