The Community Health Worker Capacity Building Collaborative seeks to:

  • Leverage expertise across the community
  • Develop the best possible Community Health Worker recruitment and programming efforts possible. 

Our leadership and teams support this effort and vision. 

The Leadership team is responsible for overall project activities. 

  • Overseesr the development and implementation of training programs
  • Meets weekly to review progress, develop next steps and address barriers to recruitment, implementation and sustainability. 
  • Approves all grant activities.

To achieve the goals of this program, we will establish a Community Health Worker Advisory Board (CHWAB) and have recruited academic, community-based, other federal agencies, and non-traditional community organizations that foster integrated training experiences.

The Board will consist of all external stakeholders from all our partner institutions, CHW students, current CHWs, and other health support workers and will review, advise, and disseminate grant activities and products. The CHWAB will meet semi-annually at a minimum.

This team will be led by members from the academic partners and the content leads from the inter-professional education team, the clinical upskilling team and the provider training teams.

The clinical provider training, upskilling, public health and inter-professional training programs proposed below are directly linked to the needs of our community health needs assessment: eliminating structural racism, enhancing trust and trustworthiness across sectors, people, communities, addressing community conditions, such as reducing poverty and its effects, enhancing mental health and reducing substance abuse and reducing chronic illness and its effects.

The Data and Evaluation Team is responsible for the design of all evaluation tools and collecting and reporting of grant metrics. 

This includes trainee recruitment, program completion, certification, and employment.  This team will collaborate with the leadership team to disseminate grant products and outcomes to our stakeholders.  

The Interprofessional Educational team will address microaggressions in the workplace, and introduction to quality improvement and team building methods.

The Provider Training Team will develop a provider training program designed to train providers and their teams on how to effectively use CHW/HSW.

This will be designed in the train the trainer format. Additionally, these providers will participate the inter-professional training alongside the CHW/HSWs.   

The recruitment teams will be responsible for the recruitment and training of new CHWs and help with field placement and trainee retention.

These teams will develop and implement a recruitment process and the DET will collaborate with them on how best to report metrics.
