There are several options to request your medical records, and we look forward to fulfilling your request.
Download or Send Your Medical Records from MyChart
Records will be released within a few hours.
- Login to MyChart
- Go to menu “Sharing Hub”
- Select the person you want to share your health information with.
- Follow the prompts to either download or send your records.
Download or Send Medical Records for another MetroHealth patient from MyChart
Records will be released within a few hours.
- Login to MyChart
- Switch to the other MetroHealth patient’s “proxy” account
- Go to menu “Sharing Hub”
- Select the person you want to share the proxy’s health information with.
- Follow the prompts to either download or send the proxy’s records.

Request a copy of medical records
Request online through MyChart
- Login to MyChart
- Go to menu ‘Sharing Hub’
- Select ‘Request formal copy of health record’
- Fill out the online form. You will be asked to determine which records are to be released, and whether to release them to yourself or directly to your provider.
You can also use MyChart to request radiology images to share with your health care provider(s). Learn more.

Request an Electronic Health Information (EHI) Export
Please complete the form provided below to request an EHI Export.
This export is provided as a .zip file, of all information within your entire medical record. This large file takes up a lot of space and can only be read by a software application. Each time an EHI Export is requested, the system will capture everything in your medical record at the time it is requested.
If you need a copy of your medical record for yourself or a medical provider, consider using the request process above instead.

Medical Records Frequently Asked Questions
Download, complete, sign and date the Authorization to Release Protected Health Information to Another Facility (en español) and return via email, fax or mail. Please note that unsigned or undated requests will not be processed.
There are situations where there are charges for medical records. The State of Ohio, in House Bill 331, has established a fee schedule. See the fee schedule.
You may request copies of your own medical records or those of a minor child, or someone for whom you are the legal guardian or hold a Power of Attorney for healthcare. A copy of the guardianship or Power of Attorney for healthcare must be provided.
Spouses, children or the executor of the estate may request medical records for deceased patients. A copy of the death certificate or executor of the estate must be provided.
It generally takes up to ten business days to fulfill your request due to the large number of requests we receive.
No. We cannot fax medical records due to confidentiality issues.

The attorney should include payment for the minimum $15.00 charge for the bill copy. The balance, if any, will be billed to the attorney.
