“Graduation” to Primary Care for Diabetes – creating a workflow to formalize a handoff
of well controlled diabetic patients from endocrinology back to their primary care
providers
Improving the accuracy of smoking history for lung cancer screening – An intervention to identify patients whose smoking history may be underestimated and prompt their primary care or pulmonology providers to update it with the goal of
increasing lung cancer screening.
This was tested this as a randomized QI initiative, and found to show that intervention significantly increased rates of low dose CT scan ordering among these patients.
Implementing A-F bundle in ICUs with updates and standardization of ICU documentation and sedation practices.
