Refer a Patient

To refer a patient, call 216-778-4700 or fax 216-778-2700.

Required Paperwork

To begin the referral process, please fax the following information to 216-778-2700.

  • The patient's medical record number (if coming from The MetroHealth System)
  • Diagnostic procedures performed and the results of those procedures
  • Contact information for your patient
  • Your contact information

We use this information to determine the best way to help your patients. This information also enables us stay in touch with you as the treatment progresses, so you are informed about any tests we've performed and results we've seen.

Appointment Specifics

A typical consultation appointment lasts 40 minutes, and we contact the patient in advance if there are fasting or medication requirements.

If your patients are coming to us for a specific procedure, visit our patient-facing procedures page for detailed information about how to prepare for all the tests we offer. Sharing that information with your patients helps them plan for their appointment.

If your patient has geographic restrictions, we will attempt to schedule appointments in the facility within those boundaries. Please note that equipment location often dictates where an appointment is scheduled.
