April 6, 2020 Update: Due to the emerging public health impact of the coronavirus pandemic (COVID-19), the June 2020 ACHI Conference will be postponed until summer of 2021. More details will be made available in due course.

The MetroHealth System is proud to partner with the AHA as a sponsor of the Association for Community Health Improvement's (ACHI) 2020 National Conference, in Cleveland, Ohio. The conference brings more than 700 community and population health leaders together to approach community health and health equity through one lens, sharing innovative strategies and new tools to tackle tough challenges — and boldly transform how communities and populations experience health and care.

MetroHealth will be presenting a series of “How To” workshops and panel discussions to provide attendees with a road map for leading, planning, implementing and funding work to address the social determinants of health. The following, hands-on workshops will be featured:

A. Building the Foundation for Success: Establishing a structure, systems and staffing models that work to propel your social determinant initiatives forward.

B. You Can’t Do It Alone: Collaborating with partners to determine, develop and measure programs that will make an impact now and long-term.

C. Find the Funding: Explore creative funding options from corporate philanthropy to non-profits, payers and government sources.

These workshops will be beneficial for community and population health executives, providers, leaders of social and community agencies, hospital administrators, health equity, outreach and program managers.
