What is Virtual Chronic Disease Management?

MetroHealth’s Virtual Chronic Disease Management (VCDM) program works with you to get the right care, when you need it, and most importantly, where you need it – at home:

  • Provides short term, remote monitoring
  • Regularly assesses your condition
  • Connects you with our Virtual Care Nurse Coordinators
  • Helps you manage your health condition with coaching support
  • Intended to be used from the comfort of your own home

How Does It Work? 

  • Your health care provider sends you an invitation to participate in the program.
  • After enrolling, you will receive the appropriate remote patient monitoring (RPM) device in the mail and a code to access the CareSimple app. 
  • This device takes your measurements and sends our readings directly to your electronic health record for your care team to review.
  • Your care team then monitors your health and can make informed decisions on how to provide the best care for you and your condition based on the regular readings you provide. 

About the Equipment and Mobile Application

  • With easy to use RPM equipment and the CareSimple Mobile application, information is securely shared electronically with your Nurse Coordinator.
  • The CareSimple mobile app is the communication hub between you and your caregivers and helps to provide you with a better understanding of your condition and how to manage it.

The following conditions are currently able to be monitored:

  • Hypertension/Coronary Artery Disease (CAD)

MetroHealth has partnered with CareSimple to provide state-of the-art tools to make your remote health coaching experience easy and secure – tools such as 4G blood pressure cuff, 4G and 4G pulse oximetry, 4G glucometer and a Bluetooth pulse oximetry.  Once enrolled, you’ll receive a package from CareSimple with everything you need to get started.

MetroHealth uses the special equipment to assess your condition while you are at home.  Using that information and your medical history, we provide coaching and help with finding the right care for you.

No, MetroHealth is not always looking at your VCDM information.  The VCDM program is NOT a 24-hour monitoring service and does NOT provide emergency response service.  Dial 911 if you experience a life-threatening emergency.

Patients with the following conditions are not currently eligible to participate in the program. If you are uncertain or have additional questions, call the MetroHealth Virtual Chronic Disease Management team at 216-957-3040.

  • Resides in an acute/sub-acute care setting
  • Conditions such as cancer treatment and advanced dementia or other conditions that may affect monitoring
  • Hospice
  • Pregnancy
  • End stage renal disease
  • Severe vision and hearing barriers

While we highly encourage you to complete the program, you can stop using the Virtual Chronic Disease Management program tools any time and for any reason. If you stop, you can still access other care and services from MetroHealth.

You can rest easy knowing that any information is sent securely. However, as with any telehealth initiative involving the secure sharing of medical/mental health information electronically, there is a minimal risk that an outsider could try to intercept these electronic communications, which could cause a privacy breach.  MetroHealth will do everything it can to protect your privacy.

You will be asked to return the equipment when you complete or end the Virtual Chronic Disease Management program.

You may be asked to pay for equipment that is damaged or lost. For additional information, please contact the MetroHealth Virtual Chronic Disease Management team at 216-957-3040.
