What is Virtual Chronic Disease Management?
MetroHealth’s Virtual Chronic Disease Management (VCDM) program works with you to get the right care, when you need it, and most importantly, where you need it – at home:
- Provides short term, remote monitoring
- Regularly assesses your condition
- Connects you with our Virtual Care Nurse Coordinators
- Helps you manage your health condition with coaching support
- Intended to be used from the comfort of your own home
How Does It Work?
- Your health care provider sends you an invitation to participate in the program.
- After enrolling, you will receive the appropriate remote patient monitoring (RPM) device in the mail and a code to access the CareSimple app.
- This device takes your measurements and sends our readings directly to your electronic health record for your care team to review.
- Your care team then monitors your health and can make informed decisions on how to provide the best care for you and your condition based on the regular readings you provide.
About the Equipment and Mobile Application
- With easy to use RPM equipment and the CareSimple Mobile application, information is securely shared electronically with your Nurse Coordinator.
- The CareSimple mobile app is the communication hub between you and your caregivers and helps to provide you with a better understanding of your condition and how to manage it.
