Referrals

Each referral is evaluated individually according to the needs of the patient and family.

Physicians, discharge planners and medical care managers: 

Call the Admissions Coordinator at 216-778-4167, or Fax 216-778-1120.

Please include the following in your fax:

  • Two to three days of progress notes, relevant consults
  • Patient Face sheet
  • History and physical
  • PT/OT/SLP Evaluations and progress notes within the most recent 48 hours
  • Current labs, CT scans, MRI results
  • Medication list/current MAR
