  • Review your personal information and make any changes as needed

Verify insurance information

  • Add new insurance coverage as needed

Review and sign documents from your phone

  • Sign consent for treatment as needed

Pay your copay ahead of time

  • If you have a copy due at the time of your visit you can pay ahead of time by entering your credit/debit card information

Keep your medication list up-to-date

  • Add or remove medications you are taking
  • Update your pharmacy information

Update your allergies

  • Add or remove allergies

Complete required questionnaires

  • All in person visits require a COVID-19 questionnaire
  • Complete other questionnaires as needed based on visit type
