In both inpatient and outpatient settings, our palliative care specialists provide expertise in symptom management and medical decision making. They assist in communication with chronically and critically ill patients and their families about medical treatment and possible end-of-life goals.
Throughout a patient’s care, our team provides increasing levels of support as illness progresses by addressing the physical, mental, and spiritual needs of the patient and their loved ones. A sample patient plan approach is as follows:
- Match patient care with patient goals
- Understand the physical basis of distress symptoms and manage them
- Assess patient’s need for additional professional consultation and coordinate additional care
- Ease the transition from hospital to hospice when appropriate

Collaboration with Hospice Organizations
A part of palliative care, hospice care typically helps patients during the last six months of their life. MetroHealth’s Palliative Care team works together with community-based hospices to provide patients and their loved ones with the highest possible quality of life.
Palliative care experts are introduced during the early phase of a chronic, critical, or terminal illness. They work in tandem with the patient’s primary healthcare team and coordinate care as the illness progresses. In addition, they assist with the transition to end-of-life hospice care, if needed.
