When your enrollment in MetroHealth Care Plus began, you received an identification card (ID card). It looks similar to this:

Always keep your ID card with you. It is important to have your ID card handy whenever you schedule appointments and every time you receive healthcare services. Providers will need information from that card to learn about what benefits may be covered for your healthcare needs. Without the ID card, providers may require you to pay directly for their services.
Have your ID card handy when you call Customer Service. This will help us serve you better.
Never let anyone else use your ID card. It has only been issued for your own use and does not apply to dependents.
