MetroHealth Care Plus Primary Care Provider (PCP) and Care Coordinator

As an enrollee of MetroHealth Care Plus, you must choose a primary care provider (PCP) from the MetroHealth Care Plus provider network.

MetroHealth Select: More employers are offering MetroHealth Select to their employeesEach PCP is affiliated with one of our patient-centered medical homes, which are also staffed with care coordinators to assist you with your health care needs and team with your PCP.

Your PCP will work with you to review and direct your health care, perform checkups and care for your routine healthcare needs.

If needed, your PCP will send you to other care providers (specialists) in the MetroHealth Care Plus provider network or admit you to the hospital.

If you haven’t chosen a PCP, we will assist you by assigning one for your next appointment. Our Customer Service team at 1-800-362-4934 can also provide you information about which physicians participate in the MetroHealth Care Plus network.
