Q.  How may I request a copy of my medical records?

A.  Download, complete, sign and date the Authorization to Release Protected Health Information to Another Facility and return via mail.   Please note that unsigned or undated requests will not be processed.  If you are the patient requesting a copy of your record, include a prepayment of $10.00.  Make check or money order payable to The MetroHealth System.  This prepayment will only cover the first 4 pages of your records.  You will be billed for any balance.  Please note:

  1. Specify the visit dates and documents you need.
  2. Allow 10 business days to receive copies.


Q.  Is there a charge for copies?

A.  Yes.  The State of Ohio, in House Bill 331, has established a fee schedule.  You can see the fee schedule here .


Q.  Who may request medical records?

A.  You may request copies of your own medical records or those of a minor child, or someone for whom you are the legal guardian or hold a Power of Attorney for healthcare.  A copy of the guardianship or Power of Attorney for healthcare must be provided.  Spouses, children or the executor of the estate may request medical records for deceased patients.  A copy of the death certificate or executor of the estate must be provided.


Q.  When may I expect to receive my copy?

A.  Because of the large number of requests we receive, it generally  takes up to ten business days to fulfill your request.


Q.  Will you fax the copies to my home or office?

A.  No.  We cannot fax medical records due to confidentiality issues.


Q.  Who can answer questions about my bill?

A.  Call the Financial Customer Services Department at  (216) 957- 3250.


Q.  How can I obtain a copy of my bill for a legal action?

A.  Please ask your attorney to submit a written request with a patient signed authorization to:

The attorney should include payment for the minimum $15.00 charge  for the bill copy.  The balance, if any, will be billed to the attorney.


Q.  May I obtain a copy of my x-ray films from Medical Records?

A.  No.  Please contact the Department of Radiology, Film File Room, (216) 778-4074.
