Medical Records Requesting Copies of Birth Certificates

To obtain a copy of a birth certificate for a child born at the MetroHealth Medical Center, you must contact:

The Bureau of Vital Statistics
City Hall, Room 122
601 Lakeside Avenue
Cleveland, OH  44114

You may contact their office by phone at (216) 664-2315.  You can also  request a birth certificate by going to http://www.vitalchek.com .  It may take up to two months for the Bureau of Vital Statistics to process a birth certificate for a newborn.

If you require proof of birth before the birth certificate has been processed by the Bureau of Vital Statistics or if you have questions or concerns regarding the birth certificate, please contact the following departments at the MetroHealth Medical Center: 

Births from March 1994 to Present:

Births Before March 1994:
MetroHealth Medical Center, Medical Record Department, Health Information Services

Should you wish to establish the paternity of a child, please contact the Paternity Enhancement Representative for the MetroHealth Medical Center at (216) 778-8590.  The MetroHealth System does not perform genetic testing to establish paternity.  If you require this service, you can contact Cuyahoga County Child Support Enforcement Agency at (216) 263-4500, or a private testing lab at  (800) DNA-CENTER.
