At The Senior Health & Wellness Center we have more than 20 senior health specialists - all of whom have completed a specific course of study focused on the care of elders. These physicians are experts in maintaining and improving the physical, emotional, and social well-being of seniors. The doctors at The Senior Health & Wellness Center have a deep understanding of how difficult chronic medical conditions can be.  The Senior Health & Wellness Center will include the following medical and social services for seniors:

• Primary and specialty outpatient services to address the health care needs of aging adults
• Home and community-based nursing and personal care services
• A Program for All-Inclusive Care for the Elderly (PACE) for frail adults who live alone or with family members
• A 144-bed long-term care unit with six specialized areas to ensure that residents receive the level of care that best meets their needs
• A 14-bed hospice unit supported by comprehensive end-of-life care
• An indoor resource center, coordinated by the Western Reserve Area Agency on Aging, that offers easy access to a wide range of organizations providing services to adults.
• Amenities such as a gift shop, beauty shop and barber shop, cafeteria, and a full-service pharmacy.

The Senior Health & Wellness Center is the result of a unique collaboration of four local organizations committed to providing services to older adults.

• The MetroHealth System, offering the largest group of senior health specialists in the region and expertise in skilled nursing care.
• The Visiting Nurse Association of Ohio, committed to bringing hospice care and home care to urban communities.
• Concordia Care, an innovative concept in adult day care that manages the health needs of the frail elderly and keeps them independent.
• Benjamin Rose Institute, creating best practice guidelines to improve medical outcomes.
