Comprehensive Care Program Programs and Services

The Comprehensive Care Team provides primary care to persons of all ages with Special Health Care Needs. Services include care for both acute illness and urgent medical needs, as well as routine preventive care such as physicals. Scheduling appointments is done through the Comprehensive Care office by secretaries who are familiar with you and your loved ones. An experienced RN is available at all times to answer your questions and concerns. A Registered Dietician is part of the team and is available to assist with nutritional needs both in the clinic as well as via instructional handouts and education. In addition to providing daily access to care for all patients, the Comprehensive Care Team offers several additional specialty clinics, including those for pediatric and adult patients with spina bifida, hydrocephalus, and cerebral palsy.

Specialty Clinics

You do not need to be a patient of Comprehensive Care, or even of the MetroHealth System to be seen in these clinics. We accept referrals from patients themselves, as well as community providers. Following completion of the clinic, a detailed report is sent to each patient, their Public Health Nurse — where applicable — and, if they are not a Comprehensive Care patient, to the patient’s primary care provider.  This report explains all that was done, as well as recommendations from each discipline that evaluated the patient during the clinic. We recommend patient’s be evaluated in this clinic about every 6 months.

This program is an on-going day to day part of Comprehensive Care. Each patient is assigned an RN whom is a go-to person for all questions, concerns and assistance with any issues related to their care.

In addition, while Comprehensive Care sees patients throughout the life span, not all specialists do so, and some transition care to adult providers as the patient turns 21 yrs old. The Care Coordinators can assist with this transition, facilitating an efficient and seamless transfer of care. The Comprehensive Care team can remain the base for care and on-going coordination between all the specialists.
